How to Register

When joining the practice, all patients will need to complete a registration form. Registration forms are available from reception or you can download them by following the link in the pre-registration section below. Following completion, you will need to provide us with some photo ID and proof of address, you will also be required to attend for a new patient medical with one of our nurses.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Please telephone the surgery and book an appointment with the Practice Nurse for a New Patient Medical (one appointment per patient). When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and asked for your photo ID and proof of address. We do like to keep a photocopy of your ID, until your registration is accepted by NHS Warwickshire. In order for us to do this this we will ask you to sign a consent form. If your registration is then rejected for any reason, we can use this information to correction the reasons for the rejection (they are often spelling erros etc). As soon as your registration is accepted your copy of ID will be securely disposed off (shredded on site). If for any reason you do not want us to retain a copy of your ID and should your registration be declined we will need to contact you, to provide any further clarification/proof. The normal registration process is completed between 2-6 weeks. 

Pre-registration Form

When you visit the surgery for the first time you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. As the form is detailed you can fill this out in advance using the our online version, which you can send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Child Health Surveillance

This ensures that your child is called for all immunisations and developmental checks. This will automatically be applied when you register your child with us.
